78 Hoylake Crescent is a small, welcoming residential care home providing personalised, person-centred care in a safe, comfortable and homely environment. With a maximum of four residents, we are able to offer a more personal approach to care, taking time to understand each individual’s needs, preferences, routines, abilities and personal goals. We support adults with a range of needs, including mental health conditions, learning disabilities, dementia, physical disabilities, reduced mobility and more complex support needs. Support for people living with dementia is provided subject to an individual assessment to ensure that we can safely and appropriately meet their needs. We offer both long-term residential placements and short-term respite care. Long-term placements provide stability, continuity and a familiar environment for people who require ongoing support. Respite care can provide temporary support following a hospital stay, during a family carer’s break, or when someone requires additional care for a shorter period. Our support is tailored to each individual and can include personal care, medication support, mobility assistance, meals and nutrition, continence support, emotional wellbeing, healthcare appointments, daily routines, activities and access to the local community. For residents with mental health needs, we aim to provide a calm, structured and supportive environment. We work closely with families, social workers, community mental health teams, GPs and other healthcare professionals to ensure that residents receive coordinated and appropriate support. For people with learning disabilities, we focus on promoting independence, choice, dignity and participation in everyday life. Support is adapted to each person’s communication needs, abilities, preferences and individual goals. For residents living with dementia, we aim to maintain familiar routines, provide reassurance and encourage independence wherever possible. We take time to understand each person’s history, preferences, communication style and daily routine so that support can be provided in a consistent and person-centred way. We also support residents with their physical health and mobility needs, including accessing healthcare services, attending appointments where appropriate, monitoring changes in wellbeing and working with external professionals when additional support is required. Residents are encouraged to remain as independent and active as possible. We support individuals to take part in meaningful activities, maintain relationships with family and friends, access the local community, attend appointments and continue enjoying the things that are important to them. Our care plans are personalised and regularly reviewed as residents’ needs change. We work in partnership with residents, families and professionals to ensure that care remains appropriate, responsive and focused on the individual. Our aim is to create a genuine home environment where residents feel safe, respected, listened to and valued, while receiving the right level of support to maintain their independence, wellbeing and quality of life. 78 Hoylake Crescent residential care home is currently rated Good by the Care Quality Commission across Safe, Effective, Caring, Responsive and Well-led.
